Ayogo Privacy Notice

Ayogo (“we”, “us”, “our”) is always working to improve the CKD LifePlan™ app (“app”) to better support users like you. In order to best prioritize our efforts, we would like to collect usage data from the app that helps us to understand how the app can be improved which may constitute personal information as defined by applicable privacy laws.

By providing personal data (as defined below) in the app, you understand that your information will be treated in accordance with this Privacy Notice and the Ayogo Privacy Policy and you consent to us using the information as set out in those documents.  If you do not want us to collect, use or disclose your personal data, you may choose not to use the app. 

You can withdraw your consent at any time, subject to certain restrictions.

What personal data is collected?

User Generated Data is data submitted by the user in the course of using the app, which is

determined and controlled by the User, which includes:

  • Name
  • E-mail address
  • Account username and password
  • Your voluntary, self-reported answers to questions, activities and scenarios posed by the App
  • Your degree of certainty or confidence regarding certain answers
  • General statements about your health condition, treatment and chronic kidney disease stage
  • Lifestyle and treatment choices, preferences and values
  • Your activities completed in the App
  • Analytics information regarding your use of the App

How will my personal data be used?

These types of data will help us to:

  • Provide the CKD LifePlan™ app
  • Develop new and improved content
  • Improve the app so it is easier to use and can better meet your needs
  • Provide customer service, like solving problems and responding to your requests

Who may receive my personal data?

Any information we may share with third parties will be aggregated at a population level and will never concern any one particular individual. Ayogo may also use approved subcontractors to process your personal data and assist with administering the App. Ayogo enters into privacy contracts with any of our subcontractors who process your personal data.

These privacy contracts ensure that our subcontractors will adequately and appropriately

maintain and process your personal data, and only use your personal data to assist Ayogo in

providing you with the App.

Your data will be accessed by your healthcare team through a web based Clinician Dashboard

who can use it to manage patients, facilitate shared decision making by reviewing patients

decision preferences and lifestyle values and to export individual patient reports. 

Will my personal data be transferred to other countries?

Ayogo is a Canadian company, but all personal information is stored in a secure cloud storage

service in the European Union using industry best-practice technical and organizational measures to prevent unauthorized or unlawful processing of personal data and accidental loss or destruction of, or damage to, personal data. Additional best practices such as carefully managed access to root passwords and secure shell (SSH) keys are used to ensure that all appropriate measures are taken to provide data security.

How long will you keep my personal data?

Your personal data will only be kept for as long as we reasonably consider necessary for

achieving the purposes set out above, or for as long as we are legally required to keep it.

How does Ayogo secure my personal data?

We take your privacy very seriously and employ administrative, physical and electronic measures to safeguard your information from identity theft and unauthorized access. We cannot, however, ensure or warrant the security of any information you transmit to us and you do so at your own risk. Once we receive your transmission of information, we make commercially reasonable efforts to ensure the security of our systems. However, please note that this is not a guarantee that such information may not be accessed, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards.

What rights do I have regarding my personal data?

You can withdraw your consent to the collection, use and disclosure of your personal data at any time, subject to our legal obligations, by contacting us in the manner set out below.  However, if you refuse to provide certain personal data or withdraw your consent, this may limit our ability to provide you with services.

Subject to certain limited exceptions, you can request access to your personal data, and request correction by contacting us in the manner set out below.  Access may be subject to a fee.
